Reimbursement application forms have been mailed to all producers who designated tobacco in 2000.  If you believe that you should have recieved a form and didn't, call 919-821-4560 and request that the forms be mailed to you. Application and Stabilization membership forms are available for download below. The forms will be downloadable only in PDF format.
